2011-2012 Season Registration
IMPORTANT:
Registration for the 2011-2012 season is ongoing.
If people register after the August 10th they need to be aware that their child will not be allowed on the ice until the Registrar has had the opportunity to process their application (insurance has to be in place). The Registrar is away until late August and it may take a few days to process any new registrations.
Please get your registration in as quickly as possible so that your child does not miss any ice time!
Please note:
Players have until June 30th to register. After June 30th, a $50.00 late fee will be applied.

Please fill out ALL the Registration and Medical Forms completely (located below).
Then mail the forms with your cheque for registration & the $50 volunteer fee (unless you commit to and sign-up for a volunteer position at one of the fundraiser events this year) to the Registrar at the above address.

Extra Forms and Information:
Application for Player Movement
Player movement forms are to be used for players who have moved from one minor hockey association to another.
If you would like your child to play a division level up (or down) within the SCMHA, please submit a letter to the President of the SCMHA outlining your situation and reasons for wanting your child to play up or down. Decisions will be made on a case-by-case basis with coach, parent, child and Association input after registration numbers are in.
Note: The hockey executive voted to allow families with 3 or more children enrolling in hockey a $150.00 discount. Contact the Registrar for more information.
